Help Us to keep your child’s vaccine record up-to-date!

The American Academy of Pediatrics recommends all children ages 5 + receive the COVID-19 vaccine. Many of our families have begun the process of getting vaccinated throughout the different community vaccine distribution sites. It is very important that we keep all our patient’s immunization records up-to-date.

If your child has received both COVID-19 vaccines and is considered fully vaccinated, please send us a picture of the verification card through your portal. Please include in the message, the dates received since the cards can be difficult to read when uploaded.

If you have an upcoming scheduled office visit, you can bring a picture of the card with you at that time if you prefer.
