Introducing a New and Easier Way to Complete Your CHADIS Questionnaires!
To provide you and your child with the best care possible, we need you to complete CHADIS before each visit. Your CHADIS questionnaires will now be integrated with our Electronic Health System (PCC) making it possible to access CHADIS through your patient portal. You no longer have to log in to a separate CHADIS account.
- You will receive email or text requests to login to your PCC portal and complete the questionnaires in CHADIS.
- Please click the link in the email or text to log in to your PCC portal.
- Click the orange “Start Questionnaire” button to complete the questionnaires before your appointment.
Important Information for Teen Questionnaires
If your child is 13 years or older you will receive email or text requests to login to your PCC portal and complete the questionnaires assigned to you. Once you have completed your questionnaires, please follow the instructions in your portal to invite your teen to log in to CHADIS and complete their questionnaires.
