We are thrilled you have chosen New Canaan Pediatrics to partner with you in caring for your family and we look forward to lots of happy and healthy years ahead!
Whether your family has recently moved to the area or if you are in need of a new pediatrician, we want to make the transition as smooth as possible. Having a well-visit as your first visit is ideal in that it allows us to get to know your child when he/she is feeling good. However, if you need to be seen right away, we can make that happen.
We will need your child’s records and immunizations.
We will need your child’s records and immunizations prior to the first visit. If your child has no medical concerns, we will need the past three well-visits and the most up-to-date vaccination record. If your child does have some medical concerns we need all the information about those concerns. If he/she sees a specialist, we would need notes from those visits.
You or your current doctor can fax records to 203-801-2127, mail, or drop off at our front desk. Once we receive the records and our registration, we will call you to set up your first appointment.

During your initial appointment, we will explain everything in detail and answer any questions you may have.
